Meaning

The "filename" attribute of every "EmbeddedDocumentBinaryObject" (attachment, BT-125) must be unique.

Possible causes

• Two or more attachments share the same filename.

How to fix it

• Give each embedded attachment a distinct @filename value, e.g. "attachment-1.pdf", "attachment-2.pdf".
